Student Registration/Transfer

 

Please see the WS/FC Schools website for answers to common enrollment questions. To locate the appropriate school for your address you can call any WS/FC school or go to the WS/FC Schools website and click on the Departments tab in the blue banner, then choose Student Assignment from the alphabetical list, then click on School Locator.

 

Please Note: All Sixth grade students on or after August 1, 2008 are required to have one booster dose of Tdap (tetanus/diphtheria/pertussis) if the last dose/shot was more than five years ago. Students that have not provided proof of immunizations, after 30 calendar days of enrollment, will be suspended from school until proof is provided.

 

All students registering at a WS/FC school must be accompanied by a parent or legal guardian.

 
Legal Custody: If a child's parents are not deceased or have not abandoned the child, the transfer of legal custody is the way a person acquires parental authority and responsibility in North Carolina. Custody may be transferred by written agreement or court order. The agreement must be signed by the relatives or family friends acquiring custody and by both parents, unless a parent is deceased or abandoned the family and left no forwarding address. The agreement must be permanent, not temporary, to enroll the child in school. The agreement should contain sections dealing with visitation rights of non-custodial parents, child support and medical care. It is recommended that people consult an attorney to assist them in drafting the agreement. People also can obtain information about drafting a child custody agreement at the public library or on the Internet. The school system will not assist a parent or relative in drafting a child custody agreement.

 

There are many situations in which a grandparent, aunt or uncle or other relative provides a home for a child. In order for that person to enroll the child, he or she must produce one or more of the following:

* A court order granting legal custody.

* Adoption papers.

* Guardianship papers signed by a judge or clerk of court.

* Caretaker affidavit(s). This form must be NOTARIZED

 

Caretaker Affidavit/Parent or Guardian Affidavit: North Carolina allows a person, other than a student's parent, to enroll a child if certain specific conditions exist by completing a Caretaker Affidavit by the person assuming custody of the child, and a Parent or Guardian Affidavit by the current person having legal custody. In general, two affidavits must be completed, one by the caretaker and one by the parent, unless the parent is decreases or so physically ill or disabled that he or she cannot complete the affidavit. If this situation exists, the Caretaker should provide documentation to demonstrate their claim as a caretaker.

 

The conditions that apply for the use of a Caretaker Affidavit are as follows:

* The death, serious illness or incarceration of the parent or legal guardian. * The abandonment by a parent or legal guardian of the complete control of the student as evidenced by the failure to provide substantial financial support and parental guidance.

* Abuse or neglect by the parent or legal guardian as evidenced by some report from the appropriate social service agency where the student previously was domiciled or resided.

* A serious illness or physical or mental condition of the parent or legal guardian is such that he or she cannot provide adequate care or supervision of the student.

* The relinquishment of physical custody and control of the student by the parent or legal guardian upon the recommendation of the Department of Social Services or the Division of Mental Health.

* The loss or uninhabitable condition of the student's home as a result of a natural disaster.

* The parent/legal guardian is assigned to active military duty and (1) is deployed outside of Forsyth County or (2) the student comes to Forsyth County to reside during the period of active duty of the parent/legal guardian.

* The parent/legal guardian lives in Forsyth County, NC, is leaving the County temporarily and plans to return within a period of a few months or less.

 

Caretaker affidavit forms may be obtained from any public school in Forsyth County. The affidavits must be signed and notarized. If you knowingly make a false statement in an affidavit, you are subject to criminal prosecution.

 

Transferring from outside the WS/FC School system:

Please bring with you:

(A) Proof of residence (current utility bill, rent agreement) A drivers License or cable bill are not considered proof of residence.

(B) Contact information for last school attended

(C) Report card/check out sheet from the last school

(D) We will request immunization records from the last school attended, however if the record is not on file, we will ask that you contact your doctor for compliance with state law. State law requires children to have had 4 or 5 DPT shots (one after the fourth birthday), 3 or 4 oral polio vaccine doses (one after the fourth birthday, 1 Hib shot (on or after the first birthday or after July 1, 1994), and 2 MMR shots (first dose on or after the first birthday). A recent change to state law requires students to have a booster dose of Tdap (Tetanus/diphtheria toxoid) vaccination if more than five(5) years have passed since the last Tdap shot.

 

Special Education: Students who have been receiving special education services should contact the Department of Exceptional Children at 727-2083 before enrolling.

 

New students who move to Forsyth County and indicate that English is not the first language the student learned or the language spoken most often at home, or is not the language they speak most; or if they are not fluent in the use of the English language, or whom have been receiving English Second Language services at their last school, need to be screened at the Newcomer Center prior to registration at a WS/FC school.

Transferring Students from another WS/FC School

You must check-out of your current school prior to registering at Hanes.

Families who will be changing their domicile by November 1 may be assigned at the beginning of the school year to the school serving their new domicile, after giving documentation to the new school.

Parents changing domicile within the county or moving outside the district may keep their child at their current school for the remainder of the school year only. Transportation is the parent's responsibility.
 

Please bring with you:

(A) Check-out sheet from the former school.

(B) Proof of residence (current utility bill, rent agreement) A drivers License is not considered proof of residence.
 
Transfer
 
If you move during the school year, it is best to let the school know as soon as possible.  On the students last day of school, they will pick up a Check Out sheet in the Guidance Office.  The student will take the Check Out sheet to all their teachers, the Media Center and the Cafeteria for signatures.  Once completed the student will return the Check Out form to Guidance and we will give the a copy to take to their new school.
 
If you move within Forsyth County, you have the right to keep your student at Hanes for the remainder of the school year.  Transportation may or may not be available from your new address, however.
