• Welcome to Mount Tabor High School!

    The first step to registering for High School in Winston-Salem Forsyth County is to verify the high school zone in which you live. You can verify this at School Locator. If you are in the Mount Tabor zone, please call the Counseling Center at 336-703-6728 to make an enrollment appointment.  An enrollment packet will need to be completed and a parent/legal guardian must be present for the enrollment appointment. 

     

    Items that are REQUIRED BEFORE registration can begin are listed below:

     Students NOT currently attending a Winston-Salem/Forsyth County Public School:

    1. To establish residency – 1 of these 3 items must be submitted:

     

    • Utility Bill – Electric, Gas or Water (current with 30 days). The address where you reside must match the service address. Disconnection notices will not be accepted.

     

    • House Rental Lease Agreement – current dated lease in parent/legal guardian’s name with occupants listed.

     

    • Mortgage Statement/Tax Record – in parent/legal guardian’s name

     

    IMPORTANT NOTE: If the parent/legal guardian does not have one of the above items in their name a Residency Affidavit (form provided by the school) must be signed by owner of the property and parent/legal guardian in the presence of a Notary and notarized. The owner of the property must provide proof of address in their name, as stated above.

     

    2. To establish Legal Guardianship:

     

    •  Student’s Birth Certificate

     

    • Valid Photo ID of Parent/Legal Guardian

     

    • Custody Agreement (if applicable)

     

    • Court Signed Documents (if applicable) If you are not the student’s parent, you must provide a court-ordered signed document indicating that you have legal custody of the student.

     

    3. To determine proper Grade and Course placement:

     

    • Transcript/Most Recent Report Card

     

    • Withdrawal Form from previous school

     

    • IEP or 504 accommodation plan information (if applicable)

     

    4. To establish North Carolina Immunization/Health Assessment Laws:

     

    • Immunization Records

     

    • NC Health Assessment Form (N/A if student has been in another NC Public School)

     

     

    Students currently enrolled in a Winston-Salem/Forsyth County Public School but moving/transferring into the Mount Tabor High School zone.
