Requests for work accommodations will be reviewed pursuant to the Americans with Disabilities Act (ADA). Under the ADA, an individual with a disability may request a reasonable accommodation to assist him or her in performing their job. Requests for support during the transition should first be addressed with the principal or supervisor. The Employee Assistance Program (EAP) is available for any employee in need of additional mental or emotional support during this time. The Web ID is COM589 or you can call 800-272-7255.
These requests must be submitted to Human Resources and will be reviewed on an individual basis. Employees must contact HR and complete the new ADA Accommodations Request Form, even if they previously completed the survey and/or temporary accommodations form. HR will also provide an ADA Physicians Request Form for the employee to have their doctor complete along with additional medical documentation, as needed.
Human Resources and Legal will review the documentation and engage in an accommodation review process. Human Resources will then send an Accommodations Response Form to the employee.
