As we plan and prepare for our student transportation needs and a safe return to school, please note these forms are crucial.  There are many challenges with conducting student screenings at school bus stops. WS/FCS will be screening all students upon their arrival at school, whether they use school transportation, are a car rider, or walker. The StrongSchoolsNC toolkit provides guidance for this process and requires school districts to administer a Parent Attestation Form for School Transportation users. 

Parents are required to complete the form below 1 time per quarter to attest to monitoring their child’s symptoms at home prior to boarding school transportation.  This is an effort to protect the health and wellbeing of their child and others.  This will also allow the school screening process to run smoothly as these students can be screened quickly and then promptly report to class daily.  We thank you for your cooperation and understanding.
