Please complete the Senior Exit Form as soon as possible.  All seniors are required to complete this form before the end of the school year.  Our office uses this form to 

1. Indicate where the final transcript should be sent after graduation (list the complete name of the college/university you will be attending and the mailing address)

2. Determine recognitions for West Forsyth Awards Day 

3. Create a comprehensive annual report of the total amount of scholarships earned by the Class of 2023

4. Maintain contact information for alumnae events
