Eletronic copies of the Diabetes Care Plan (DCP) for students with diabetes are sent to each school nurse in late April/May. You can find an electronic copy here as well:
Diabetes Care Plan. Students and families are required to have the DCP completed on an annual basis to ensure school staff can appropriately assist a child in day-to-day occurances, as well as during a diabetic emergency.
