To use networked resources, all students must sign and return this form, and those under age 18 must obtain parental permission. The activities listed below are not permitted:
- Sending or displaying offensive messages or pictures
- Using obscene language
- Giving personal information, such as complete name, phone number, address or identifiable photo, without permission from teacher and parent or guardian
- Harassing, insulting or attacking others
- Damaging or modifying computers, computer systems or computer networks
- Violating copyright laws
- Using others' passwords, logins, and/or usernames
- Trespassing in others’ folders, work or files
- Intentionally wasting limited resources (i.e. printing)
- Employing the network for commercial purposes, financial gain, fraud or illegal activities.
Other expectations are listed in the Winston-Salem/Forsyth County School System Board Policy (AR 6161.1) for Acceptable Use of Internet and Websites.
It is understood that access to the Internet through the Winston-Salem/Forsyth County School network is a privilege that is to be used for educational purposes. We have read and understand the Winston-Salem/Forsyth County School Student Network and Internet Acceptable Use Guidelines and agree to abide by these guidelines. We have discussed appropriate and inappropriate use of the computer network.
It is understood that any violation of these guidelines is unethical and may constitute a criminal offense. Any violation of these guidelines will cause access privileges to be revoked and school disciplinary and/or appropriate legal action will be taken.
As part of a 21st century learning environment, teachers and students will be utilizing a variety of web-based tools such as blogs, wikis, podcasts, vodcasts, video streaming and social media applications. These technologies improve student communication and collaboration skills, provide an authentic audience, and extend learning beyond the classroom walls while building digital citizenship skills.
I give Winston-Salem Forsyth County Schools permission to create and maintain web application accounts for my child on my behalf. I understand that Winston-Salem Forsyth County Schools has access to all usernames and passwords and can disable or shut down an account at any time for inappropriate use. I also understand I can request access to these accounts at any time.
I understand that my student is allowed to use their personal electronic device (laptop/tablet/mobile phone) at school. WSFCS is not responsible for any damage, theft or data loss, or other associated costs of replacement or repair incurred during the school day or at home as a result of being permitted to use the device at school. The student named above will take full responsibility for the device and will appropriately secure all devices when not in use. (See Policy 5131 – Student Behavior)
Although the Winston-Salem/Forsyth County School System provides Internet access that is constantly filtered and supervised, it is understood that there are text and graphic files available on the Internet which are inappropriate for minors. It is also understood that the student is ultimately responsible for his/her actions and the school system will not be held responsible for materials which the student might access.
