To participate in middle school sports, students must be examined by a physician. Fill out the student information, insurance, and concussion portion of this form. Then, take the form to your physical appointment to have the exam portion filled out by your health care provider. After the physical is completed, turn in the form to the athletics director. Sports physicals are good for 1 calendar year.

Parents/Guardians, fill out this form and turn it in to the athletics director prior to tryouts. This form and the Middle School Sports Physical form must be turned in prior to tryouts.

Student-athletes must be medically covered in case of an injury, and may not participate in sports unless there is proof of insurance. If your student-athlete is not covered under a health insurance policy, you may download this form in order to cover them for the year in case of an accident or injury.
