Wake Forest Baptist Health will be offering FREE sports physicals on MAY 30th 2020 at its SPORTS MEDICINE STRATFORD  location (1901 Mooney St, Winston Salem, NC). Atkins Time Slot will be at annouced when it becomes available.  The physical packet may be downloaded here.
