Fall 2025 Exam Week Early Check-Out Permission Form

If your student will leave campus after completing their exam, a parent/guardian must provide permission using this form.

Important reminders:

Students may only check out after their scheduled lunch time.
No calls will be made into classrooms during testing.
A parent/guardian phone number is required for verification.

Please complete one form per student.

Fall 2025 EOC Retesting Form

Students who scored Not Proficient on their EOC are eligible for a retest.
Please complete this form to confirm whether your student will participate in the retest.

Retesting for Biology, Math 1, and Math 3 will take place during exam week.
English II retesting will occur in early January when scores are returned.
Retesting is optional and only the higher score will count as the 20% exam grade.

Please submit one form per student.
