Did You Know:

  • Any student new to NC public schools must have a physical exam within 30 days of enrollment.

  • Students must have required health assessments and/or immunizations completed by 30 calendar days from the child's first day of attendance.

  • Students who do not have the required health assessments and/or immunizations completed by the 30th day of school will not be allowed to return to school until these requirements are met and the needed forms are returned to school.

  • Children entering Kindergarten must have two doses of varicella (chicken pox) vaccine, one dose of polio vaccine after age 4, and a kindergarten physical signed by their physician.

  • 7th graders must have one booster does of Tdap (after age 10) and one dose of meningococcal (MCV4) vaccine.

  • 12th graders must have one booster dose of meningococcal (NCV4) vaccine.
