Did You Know:
Any student new to NC public schools must have a physical exam within 30 days of enrollment.
Students must have required health assessments and/or immunizations completed by 30 calendar days from the child's first day of attendance.
Students who do not have the required health assessments and/or immunizations completed by the 30th day of school will not be allowed to return to school until these requirements are met and the needed forms are returned to school.
Children entering Kindergarten must have two doses of varicella (chicken pox) vaccine, one dose of polio vaccine after age 4, and a kindergarten physical signed by their physician.
7th graders must have one booster does of Tdap (after age 10) and one dose of meningococcal (MCV4) vaccine.
12th graders must have one booster dose of meningococcal (NCV4) vaccine.
