Senior Exit Survey

Seniors - it's that time of year to inform Student Services of your plans after graduation.  We must have this form to submit your final transcript to the college, employer, or military branch after you graduate.  Submit the 2025 Senior Exit Survey as soon as possible to make sure your final transcript is sent to the correct location.  We also ask that you list ALL scholarship awards earned from every college/university to which they applied - if you have not already provided that information. 

Our goal is to send final transcripts to the one college/university you will attend by June 13.
