If you are unable to set up a Parent Portal Account following the instructions on the West Forsyth Website, you may complete this Report Card Request Google Form beginning on Wednesday, October 21:
The parent/guardian must complete the entire Google form.
Only the Parent/Guardian may pick up the Report Card with a photo ID (no students may pick them up).
Report Card paper copies will be available in the Student Services Department two school days after the Google Form has been submitted.
